DATA SUBJECT EXPLICIT CONSENT FORM
Within the framework of the Clarification Text and the Personal Data Protection and Processing Policy made available to me in accordance with the Personal Data Protection Law No. 6698 (the “Law”) and related secondary legal regulations by Bes Yapı Ürünleri San. AŞ. (“Bes Yapı”, “Company”); I hereby give my explicit consent to the processing of all kinds of my personal data, special categories of personal data, and personal health data—including those provided by me verbally, in writing, and/or electronically that identify or help identify my identity—by Bes Yapı; to the sharing of such data with other persons and organizations where sharing is required under relevant legislation for business needs and for providing and planning health services; and to the processing of the personal data and special categories of personal data detailed below within the scope defined in the “Personal Data Protection Law” No. 6698 and the “Regulation on the Processing and Privacy of Personal Health Data”[cite: 4]:
- Identity Information: Data clearly belonging to an identified or identifiable natural person; processed partially or fully automatically, or non-automatically as part of a data recording system; containing information on the person’s identity, such as documents like driver’s license, identity card, and passport containing full name, T.C. Identity number, nationality info, mother’s name, father’s name, place of birth, date of birth, gender, as well as tax number, SGK number, credit card details, signature information, vehicle license plate, etc[cite: 4].
- Contact Information: Data clearly belonging to an identified or identifiable natural person; processed partially or fully automatically, or non-automatically as part of a data recording system; such as phone number, address, email address, fax number, IP address, etc[cite: 4].
- Physical Space Security Information: Personal data regarding records and documents collected upon entry to and during the stay inside the physical space; camera records, fingerprint records, and records taken at the security checkpoint, clearly belonging to an identified or identifiable natural person, processed partially or fully automatically or non-automatically[cite: 4].
- Visual and Audio Records: Photos and camera records (excluding records under Physical Space Security Information) and data contained in document copies that include personal data, clearly belonging to an identified or identifiable natural person[cite: 4].
- Special Categories of Personal Data: Data specified in Article 6 of the Personal Data Protection Law (e.g., health data including blood type, biometric data, etc.), clearly belonging to an identified or identifiable natural person, processed partially or fully automatically or non-automatically[cite: 4].
- Request / Complaint Management Information: Personal data regarding the receipt and evaluation of any request or complaint directed to Bes Yapı, clearly belonging to an identified or identifiable natural person, processed partially or fully automatically or non-automatically[cite: 4].
I confirm that I have read and understood the Information Text and my rights available on the website[cite: 4]. I acknowledge that I have been properly informed in accordance with Article 10 of the Law regarding the recording, processing, storage, and transfer of my above-mentioned personal data and special categories of personal data during the continuation of the business relationship and, in any case, for the legal retention periods set out under the relevant Law; as well as the deletion, destruction, or anonymization of my personal data upon the expiration of the legal retention period in accordance with the Law and secondary legal regulations[cite: 4].
